Enterprise Rent-A-Car Rental Reservation Form

I am aware of the established daily rental rate and other fees, and will follow proper procedures to have the costs for my rental paid through my division or department.

I shall return the vehicle on the pre-arranged date or I will notify the Enterprise branch personnel if a new return date is required.

I shall return the vehicle to the same location unless other arrangements have been made with Enterprise Rent-A-Car.

I shall abide by all state and federal laws governing motor vehicle operations and drivers licensing regulations, including, but not limited to, having a valid driver's license in my possession. Upon pick up of the vehicle I will show my valid driver license as identification.

I shall return the vehicle full of fuel, free of litter or debris and smoke free. I am aware that I will be assessed a fee in the event the vehicle is not returned in the above-mentioned condition.

I am aware that my division or department is liable for the cost to repair any damage to the vehicle due to neglect or abuse while in my possession.

I shall immediately notify Enterprise and fill out the State of Utah accident form if the vehicle is damaged in any way.

I shall follow all policies or rules published by the Division of Fleet Operations governing state vehicle use. I further understand that I am not allowed to use a rental vehicle for commute or personal business without proper authorization.

I shall not take the vehicle out of state without filling out a FI-5 form (out-of-state travel authorization form).

I have taken the Risk Management approved defensive drivers training within the last two years.

I agree to follow the policy in regards to operating Large Passenger Vans as indicated by R27-3-12 found at this link: https://rules.utah.gov/publicat/code/r027/r027-003.htm#T12 I also agree to take the large passenger van training as required by Risk Management.

I acknowledge that to rent vehicles from Enterprise (excluding full-size vans) that the minimum age requirement is 18. (Minimum age for full-size vans is 21.)

I understand that no more than 9 people, including the driver, are allowed in full-size passenger vans.

I understand as of July 1, 2011 that no fuel card will be provided with any reservation.
